At the end of each month, you will download that month's Student Mentor Report (SMR) below. This report will ask questions about your ministry assignment, your personal times with the Lord, what you are learning, and how you are feeling about it. Once complete, save your SMR and send it to your Mentor to review at your next meeting. You'll also submit your SMR by uploading it on this page before the due date.

Your Student Mentor Report (SMR) is due by the 7th of the following month (e.g. the September SMR is due October 7th)

Reports are course specific and are available for you to download below. Once downloaded you can fill them out on your computer, and upload the file here. You may also print the report and fill it out by hand, scan the document and upload it here as well.
