MENTORING MEMORANDUM OF AGREEMENT

This document is to be reviewed by both the Mentor and the Student at their first meeting to clarify what is expected by both parties. It explains what ministry assignments will be laid out for the student, when and where the mentoring meetings will take place and the reports that will be turned in by both the student and the mentor.

After going over the document, both the student and the mentor will sign the agreement. The Student will return the signed document to CVSOM by either, bringing it to class or scanning it and uploading below.
