At the end of each trimester, you will download that trimester's Mentor Trimester Report (MTR) below. This report will ask questions about your student's ministry assignments, how many times you met with your student, how your student is connecting with the subjects being taught, and basic evaluation questions. Once complete, save your MTR and send it to CVSOM by uploading it on this page before the due date.

Your Mentor Trimester Report (MTR) is due for the Fall Trimester on 12/7, Winter Trimester on 4/7 and the Spring Trimester on 6/30.

Reports are course specific and are available for you to download below. Once downloaded you can fill them out on your computer, and upload the file here. You may also print the report and fill it out by hand, scan the document and upload it here as well.
